How do I report Form 1099-DA on Form 8949 and Schedule D?

Work row by row. Each Form 1099-DA row carries an "Applicable checkbox on Form 8949" code (G, H, J, K, or Y), proceeds in box 1f for column (d), and, when the broker reported it, basis in box 1g for column (e). Form 8949 totals roll to Schedule D by box. First, though: a Form 1099-DA row records what the broker saw, not what the transaction was. Before a row goes on Form 8949 as a sale, the hash behind it needs a category. A withdrawal to the client's own wallet, or another non-disposition the broker reported, belongs in review, not on the return as a disposal. Two practical notes. First, the 2025 Form 8949 instructions refer to code X in the "Applicable checkbox on Form 8949" box for an unknown holding period, while the Form 1099-DA instructions (2025 and 2026) tell brokers to use code Y. On a Form 1099-DA, read Y as that signal. Second, covered short-term, covered long-term, and noncovered sales go on separate forms even from one trade, so one sale can produce several rows.

Exception 1 lets basis-reported rows with no adjustments skip Form 8949 and go to line 1a or 8a. Exception 2 allows an attached statement with the same detail, with summary totals on Form 8949 using code M. Firms choose the presentation.

Classification first, and the edges that stay with the firm

When a broker-reported row does not match the books, ask what the on-chain event was: a sale, a swap, a self-transfer, or something a human must settle.

The hashes that matter are around the row: lot-opening acquisitions, the deposit behind a transfer-in in box 12a or 12b, and any withdrawal the broker may have reported as a sale. An on-chain exchange of one asset for another is served as swap (house treatment disposal) when the engine resolves the shape; a shape it cannot resolve routes to needs_review. A one-way transfer whose counterparty is not verified as the client's own wallet serves as category transfer with needs_review and taxable: null (Standard GAP-1); verified self-transfers serve non_taxable. Those records tell the firm which rows to carry and which to take to the issuer. We do not write the Form 8949 row.

Worked example

A client on extension is finishing a 2025 return. A U.S. custodial exchange issued a 2025 Form 1099-DA with two rows, both code Y, box 1g blank.

Row 1: 1.50000000 ETH, proceeds $4,800 in box 1f, box 12a shows 1.5 ETH transferred in. The sale happened on the exchange's books, so there is no sale hash. The firm enters the 2023 acquisition hash and the deposit hash. The acquisition serves as swap (USDC for ETH), setting the acquisition date and basis facts. The row is a real long-term disposal. On Form 8949 it goes in Part II, box K: proceeds $4,800 in column (d), basis from records in column (e). The total rolls to Schedule D line 9.

Row 2: 0.20000000 BTC, proceeds $19,000 in box 1f. The client says they never sold BTC in 2025. The outbound hash serves as transfer with needs_review: true and taxable: null, because the destination is not yet verified as the client's own wallet. The firm confirms the address is the client's hardware wallet. That confirmation, kept in the firm's workpapers, settles the treatment; the served record stays a review-routed transfer. The row is not carried to Form 8949 as a sale. The client, with the firm's help, contacts the issuer, requests a corrected form, and keeps the original and all correspondence; the firm decides how to present the row if the correction does not arrive before filing.

Not on the form: a swap in self-custody. In August 2025 the client swapped SOL for USDC in a self-custody wallet. No Form 1099-DA exists. The swap hash serves as swap (house treatment disposal) and goes on Form 8949 in box I or L by holding period.

Do I have to list every Form 1099-DA row on Form 8949?

Not always. Exception 1 lets basis-reported rows with no adjustments go directly on Schedule D line 1a or 8a, and Exception 2 allows an attached statement instead of individual rows. Rows with blank basis or any adjustment go through Form 8949.

What does code Y on my Form 1099-DA mean?

The broker could not determine short-term or long-term. The row belongs in box H or K, and the holding period comes from the client's records. On 2025 forms, brokers use code Y whenever basis is not reported.

My Form 1099-DA shows a sale that was a transfer to my own wallet. What now?

Confirm it at the hash level, then contact the issuer, keep the original and correspondence, and do not wait to file. How to present the row if no correction arrives is a preparer decision.

Is a 2025 Form 1099-DA different from a 2026 one?

Yes. For 2025 sales, basis was voluntary (most rows blank basis and code Y). For 2026 sales, basis is mandatory for covered securities, so more rows carry G or J, and noncovered rows carry box 9.
